COMPLEX CARE

BEYOND CARE

COMMITMENTED TO WELLBEING

We believe that providing effective care for individuals with complex needs requires a holistic approach. This is why we employ the bio-psycho-social model a comprehensive framework that considers biological, psychological, and social factors in understanding and supporting mental health. This model recognises that mental health challenges are not isolated to one domain but are influenced by a range of interconnected elements. By using this model, we ensure that every aspect of an individual’s life is considered, allowing for a more personalised and effective care plan.

We utilise advanced technologies and tailored approaches to meet each individual’s unique needs, ensuring the most effective, up-to-date care.
Partnering with local authorities, we seamlessly integrate our services, ensuring coordinated and efficient care across all sectors.
Our solutions help local authorities save costs without compromising care quality, allowing resources to be reinvested where needed most.
From mental health crises to hospital care, we provide a broad range of services, ensuring complete and personalised support for every individual.
Our highly trained, NHS-standard staff bring extensive expertise to deliver exceptional care in even the most challenging environments.
We address the physical, mental, and social aspects of care to ensure comprehensive support and promote overall well-being.

The Bio-Psycho-Social Model

Biological factors refer to the physical aspects of an individual’s health, including their genetic predispositions, physical health conditions, and neurochemical imbalances. These factors play a key role in mental health and need to be carefully managed.
How We Address Biological Needs:
We work closely with healthcare professionals to monitor and manage the physical health of those in our care, ensuring they receive the necessary medical interventions and support. Whether it’s addressing physical disabilities, managing medication, or monitoring immune responses, our staff are trained to understand and work with these biological components.
Psychological factors encompass a person’s emotional state, personality, coping mechanisms, and mental wellbeing. These factors influence how individuals perceive themselves and the world around them, shaping their behaviour and interactions.
How We Support Psychological Wellbeing:
Our team uses a compassionate, person-centred approach, focusing on emotional support, coping skills, and helping individuals build resilience. By fostering self-esteem, developing positive behaviours, and teaching effective coping strategies, we help individuals manage their mental health and work towards their personal goals.
Social factors, such as family dynamics, relationships, cultural influences, and social support, also play a crucial role in mental health. These elements can either contribute to stability or exacerbate challenges, especially in cases of social isolation or poverty.
How We Foster Social Stability:
We prioritise keeping individuals within their communities and close to their families, where possible. Our services aim to strengthen social connections, address family issues, and promote integration into community life. We understand the importance of social networks in providing stability, security, and a sense of belonging.

WHY THE BIO-PSYCHO-SOCIAL MODEL WORKS

The bio-psycho-social model is highly effective for treating complex needs because it doesn’t focus solely on one aspect of an individual’s life. Instead, it looks at the person as a whole, acknowledging that mental health is shaped by a wide range of factors. By tailoring our approach to each individual’s unique biological, psychological, and social circumstances, we can create a more meaningful and impactful care plan.

THE IMPORTANCE OF SKILLED STAFFING

At 365 Care Services, we believe that successful care is not just about the individual but about the support network around them.  Placement failures often stem from a mismatch between individual needs and the skills of the care team, not from the individual’s limitations. This is why we invest in highly trained, compassionate staff who understand the nuances of complex care. Our team is equipped to anticipate challenges, manage crises, and adjust to the evolving needs of each person.

With the right staffing, we can avoid unnecessary hospitalisations, reduce crisis incidents, and promote lasting stability within the community. Our staff are not just carers they are partners in each individual’s journey towards improved wellbeing and a higher quality of life.
